The BACB Monthly Fieldwork Verification Form
The monthly form is your signed record of each month of fieldwork. Here is which of the four official versions you need, what goes in every field, and the deadline, straight from the BCBA Handbook and the BACB’s own forms.
What the form is
The Monthly Fieldwork Verification Form (M-FVF) records one month of fieldwork: your independent hours, your supervised hours, and, on the 2027 version, how much of that time your supervisor observed you with a client. You and your supervisor both sign it each month.
You don’t send it to the BACB. According to the BCBA Handbook, only the Final Fieldwork Verification Form (F-FVF) goes with your application; monthly forms may be requested if your fieldwork is audited. That makes the monthly forms the proof behind your hours, so each one has to be complete, correct, and signed on time.
The four versions, and which one you need
The BACB publishes the monthly form in two arrangements, each in a 2022 and a 2027 version:
Choosing the arrangement
- One supervisor at your organization this month: the Individual Supervisor form, signed by that supervisor.
- Several supervisors at the same organization: the handbook lets you complete either version. One Multiple Supervisors at One Organization form covers the organization; the responsible supervisor signs it and, per the handbook, must meet all supervision requirements for the entire period the form covers. Or complete an Individual Supervisor form with each supervisor.
- More than one organization: separate forms, since the Multiple Supervisors form is one form per organization. On the 2022 forms, fieldwork types are also on separate forms.
Choosing the version
The BACB’s 2027 transition resource says your monthly forms must match the requirements in effect when you apply. If you might apply either before or after January 1, 2027, it recommends completing both the 2022 and the 2027 monthly forms with your supervisor each month.
What goes in each field
Field by field, as printed on the 2027 forms (version 06/2026). The Multiple Supervisors form asks for the responsible supervisor; otherwise the two 2027 forms ask for the same things.
| Field | What to enter |
|---|---|
| Trainee Name, BACB ID # | Your name and BACB ID, as on your BACB account. |
| Month/Year | The month of fieldwork this form covers. |
| State and Country Where Fieldwork Occurred | Where you did the fieldwork that month. |
| (Responsible) Supervisor Name, Certification # or BACB ID # | The supervisor who signs this form, and their certification number or BACB ID. |
| A. Independent Hours (supervisor not present) | hh · mm |
| B. Supervised Hours (supervisor present) | hh · mm |
| These fieldwork hours include … of observation | Time your supervisor observed you with a client, in hh · mm. It’s part of the hours above, not extra. |
| Total Fieldwork Hours (add A & B) | A + B, carried into hours: 138 hh 25 mm + 11 hh 45 mm = 150 hh 10 mm. |
| Percentage of Hours Supervised (supervised/total) | B ÷ (A + B). In the example, 11 h 45 m of 150 h 10 m = 7.82%. |
| Signatures and dates | Supervisor and trainee, each with the date signed, under the attestation that the information is true and that the fieldwork met all requirements, including the BACB’s ethics requirements. |
The 2022 forms ask for the same people and totals, but in decimal hours (for example 35.50), with a box to tick the fieldwork type (Supervised or Concentrated Supervised Fieldwork) and a box for prorated hours in a partial month.
Deadline, signatures and keeping copies
- Deadline: signed by the last day of the calendar month after the month of supervision. September’s form is due by October 31.
- Signatures: the form must be signed in line with the BACB’s Acceptable Signatures Policy.
- Keeping copies: you and your supervisor each keep a copy. The forms say at least 7 years; the handbook says at least seven years after you obtain certification.
- Don’t submit it to the BACB unless they ask for it.
Common problems to avoid
The first two are stated by the BACB as reasons a form is denied or not accepted. The rest are common slips that put a month at risk.
- Missing information. The forms say forms with missing information will be denied; the 2027 forms add that incomplete documents result in the month’s fieldwork hours being lost.
- Outdated or non-BACB forms. The handbook says these aren’t accepted. Use the current official version.
- Signing late. The form must be signed by the last day of the following month.
- Filling it in a web browser. The form warns that its drop-down menus only work in Adobe Acrobat and that dates entered in a browser won’t save correctly.
- Mixing organizations on one form, or fieldwork types on a 2022 form.
- Totals that don’t match your hours. The form is your attestation of what you logged, so its numbers should come straight from your records.
How Fieldwise fills it
Fieldwise works out which forms each month needs (Individual or Multiple Supervisors, per organization, in the version for when you apply) and fills the BACB’s own PDF from your sessions. Hours and minutes are calculated exactly as the 2027 form calculates them. You can view and download every form for free; with Pro, you sign and send it, and your supervisor signs from an email link on any device.

Questions
When is the monthly verification form due?
It must be signed by the last day of the calendar month after the month of supervision. A September form must be signed by October 31.
Do I send the monthly form to the BACB?
No. Keep it. The monthly forms say not to submit them unless the BACB asks, and the BCBA Handbook says only the Final Fieldwork Verification Form goes with your application. Monthly forms may be requested if your fieldwork is audited.
How long do I keep my monthly forms?
The trainee and the supervisor must each keep a copy. The forms say at least 7 years; the BCBA Handbook says at least seven years after you obtain certification.
I had two supervisors at the same clinic this month. Which form do I use?
The BCBA Handbook lets you complete either version. With the Multiple Supervisors at One Organization form, one form covers that organization and is signed by the responsible supervisor, who must meet all supervision requirements for the period the form covers. With the Individual Supervisor form, you complete one form with each supervisor.
I worked at two organizations this month. Can I put both on one form?
No. The Multiple Supervisors form is one form per organization, so each organization gets its own form. On the 2022 forms, use a separate form for each fieldwork type too.
Should I use the 2022 or the 2027 form?
The form must match the requirements in effect when you apply. If you apply before January 1, 2027, use the 2022 forms; on or after, the 2027 forms. If you might apply either side of that date, the BACB recommends completing both the 2022 and 2027 monthly forms with your supervisor each month.
Why does the 2027 form ask for hours and minutes?
The 2027 forms record independent hours, supervised hours and observation time as hours (hh) and minutes (mm), and the total adds independent and supervised time the same way: 138 hh 25 mm plus 11 hh 45 mm is 150 hh 10 mm. Observation is part of those hours, not added on top. The 2022 forms use decimal hours, such as 35.50.
Can I fill the form in my web browser?
The BACB form itself warns against it: it should be completed in Adobe Acrobat Reader on a desktop, because its drop-down menus only work there and dates entered in a web browser may not save. Fieldwise fills the PDF directly, so every field is already complete when you open or download it.
